SUMMARY:16th Annual Illinois Statewide Transition Conference in East Peoria
DESCRIPTION:The "Stepping Stones of Transition" conference shares services 
 and resources to prepare for success in adulthood.\n\nThe statewide transi
 tion conference\, titled "Stepping Stones of Transition\," highlights the 
 opportunities and resources available for youth and young adults with spec
 ial healthcare needs as they transition to adulthood.\n\nIt will take plac
 e Nov. 4-5 at the Embassy Suites by Hilton East Peoria Riverfront Conferen
 ce Center.\n\nBoth in-person and virtual attendee options are available. T
 he in-person event will follow all necessary health and safety guidelines.
 \n\nThe conference is aimed at youth\, parents\, caretakers\, vocational p
 rofessionals\, healthcare professionals\, educators and so on.\n\nParticip
 ants are invited to discuss the possibilities for students with disabilit
 ies in the areas of independent living\, education and training\, employme
 nt\, community integration\, health care and self-advocacy.\n\nYouth and y
 oung adults with special healthcare needs will learn how to plan for their
  future\, including:\n\n 	What happens after I leave high school?\n 	How d
 o I sort through the maze of resources?\n 	What steps can I take now to pr
 epare for upcoming changes?\n 	What skills are important for me to develop

 are for Children (DSCC) may be able to fund the conference-related expense
 s for DSCC participants and their family\, caretaker\, nurse and/or person
 al attendant. These expenses include the cost of the hotel and transportat
 ion.\n\nDSCC participants should contact their Care Coordinator to complet
 e their registration or call (800) 322-3722.\n\nFamily and student scholar
 ships are also available for eligible attendees.\n\nThe Arc of Illinois\, 
 through a grant from the Illinois Council on Developmental Disabilities\, 
 administers the Consumer Involvement Program. This program provides stipen
 d funds to people with developmental disabilities and their family members
  to attend conferences of their choice that are directly related to develo
 pmental disability issues.\n\nFor more information on using the Consumer I
