SUMMARY:17th Annual Illinois Statewide Transition Conference in East Peoria
DESCRIPTION:The "Stepping Stones of Transition" virtual and in-person confe
 rence shares services and resources to prepare youth and young adults with
  special healthcare needs for success in adulthood.\n\nThe statewide trans
 ition conference\, titled "Stepping Stones of Transition\," will take plac
 e Nov. 3-4.  Both in-person and virtual options are available. The in-pe
 rson event will follow all necessary health and safety guidelines.\n\nThe 
 conference is at the Embassy Suites by Hilton East Peoria Riverfront Confe
 rence Center\, 100 Conference Center Dr.\, East Peoria.\n\n"Stepping Stone
 s of Transition" is for youth\, parents\, caregivers\, vocational professi
 onals\, healthcare professionals\, educators and others who work with yout
 h and young adults with special healthcare needs.\n\nParticipants can disc
 uss the possibilities for students with disabilities in the areas of:\n\n 
 	Independent living\n 	Education and training\n 	Employment\n 	Community i
 ntegration\n 	Health care\n 	Self-advocacy\n\nYouth and young adults with 
 special healthcare needs will learn how to plan for their future\, includi
 ng:\n\n 	What happens after I leave high school?\n 	How do I sort through 
 the maze of resources?\n 	What steps can I take now to prepare for upcomin
 g changes?\n 	What skills are important for me to develop?\n\nMore informa

 icago's Division of Specialized Care for Children (DSCC) may be able to fu
 nd the conference-related expenses for DSCC participants and their familie
 s\, caregivers\, nurses and/or personal attendants. These expenses include
  the cost of the hotel and transportation.\n\nDSCC participants should con
 tact their Care Coordinator to complete their registration or call (800) 3
 22-3722.\n\nFamily and student scholarships are also available for eligibl
 e attendees.\n\nThe Arc of Illinois\, through a grant from the Illinois Co
 uncil on Developmental Disabilities\, administers the Financial Assistance
  for Training Program. The program provides funds for people with developm
 ental disabilities and their family members to attend conferences of their
  choice that are directly related to developmental disability issues.\n\nF

 n\nThe conference also includes a health care track for pediatric\, adult 
 primary care and subspecialty providers to learn from experts and one anot
 her about the Six Core Elements of Health Care Transition and the strategi
 es\, services and resources available to help improve the quality of healt
 h care transition. The conference’s health care track is on Nov. 3.\n\nT
