SUMMARY:19th Annual Illinois Statewide Transition Conference in Springfield
DESCRIPTION:The "Stepping Stones of Transition" conference shares services 
 and resources to prepare youth and young adults with special healthcare ne
 eds and other disabilities for success in adulthood.\nThe Illinois Statewi
 de Transition Conference\, titled "Stepping Stones of Transition\," will t
 ake place in person on Nov. 7-8. The conference is at the Crowne Plaza Con
 vention Center\, 3000 S. Dirksen Parkway\, in Springfield.\nIt will highli
 ght the opportunities and resources available for youth and young adults w
 ith disabilities as they transition to adulthood.\n"Stepping Stones of Tra
 nsition" is for youth\, parents\, caregivers\, vocational professionals\, 
 healthcare professionals\, educators and others who work with youth and yo
 ung adults with disabilities. Youth will learn how to plan for their futur
 e\, including:\n\nWhat happens after I leave high school?\nHow do I sort t
 hrough the maze of resources?\nWhat steps can I take now to prepare for up
 coming changes?\nWhat skills are important for me to develop?\n\nParticipa
 nts can learn about supported decision-making from keynote speaker and sel
 f-advocate Derek Heard along with Allison Cohen Hall from the Center on Yo
 uth Voice\, Youth Choice. The conference also includes sessions on:\n\nSel
 f-determination\nAdvocacy\nEmployment\nFuture planning\nPost-secondary edu

 n\nThe Division of Specialized Care for Children (DSCC) may be able to fun
 d the conference-related expenses for DSCC participants and their families
 \, caregivers\, nurses and/or personal attendants. These expenses include 
 the cost of the hotel and transportation. DSCC participants can sign up on

 rence Steering Committee has also set up a scholarship program to help you

 amily members of a person with a disability\n\n\n\nGuardians for a person 
 with a disability\n\n\n\n\nThe amount of scholarship money available depen
 ds on each person's needs.\n\n\n\nPlease fill out this scholarship form to
