DESCRIPTION:Ask questions and explore the many options beyond high school a
 t this free\, all-inclusive transition fair for students with disabilities
  and their families\n\nThe Sangamon County Transition Planning Committee (
 TPC) and Springfield School District 186 will host the 2025 Transition Fai
 r on April 9 from 5 to 7 p.m.\n\nStudents and their families can attend th
 e event to learn about important programs and resources for success after 
 high school.\n\nActivities will include:\n\n 	Connecting with community pa
 rtners and key resources\n 	Networking with other students and families\n 
 	Speaking with representatives offering a wide range of resources\n 	A bre
 akout session with Lincoln Land Community College on navigating academic a
 ccommodations\n\nPlease stop by the University of Illinois Chicago's Divis
 ion of Specialized Care for Children (DSCC) table to learn about our trans
 ition resources and programs for youth and young adults with special healt
 hcare needs. Our team can answer your questions\, provide information\, an
 d give you a copy of our Transition Toolkit\, which is packed with helpful
  resources.\n\nThe event is free and open to transition-aged youth with di
