The co nference is at the Crowne Plaza Convention Center\, 3000 S. Dirksen Parkwa y\, in Springfield.\nIt will highlight the opportunities and resources ava ilable for youth and young adults with disabilities as they transition to adulthood.\n“\;Stepping Stones of Transition”\; is for youth\, p arents\, caregivers\, vocational professionals\, healthcare professionals\ , educators and others who work with youth and young adults with disabilit ies. Youth will learn how to plan for their future\, including:\n\nWhat ha ppens after I leave high school?\nHow do I sort through the maze of resour ces?\nWhat steps can I take now to prepare for upcoming changes?\nWhat ski lls are important for me to develop?\n\nParticipants can learn about the i mportance of disability acceptance\, awareness and inclusion from keynote speaker Taylor Sweeting. The conference also includes sessions on:\n\nArti ficial intelligence (AI) in transition\nSelf-advocacy\nEmployment\nFuture and financial planning\nPost-secondary education\nHealth care\n\nSee the c onference flyer for more details:\n\n2025 transition conference flyer in E nglish\n2025 transition conference flyer in Spanish\n\nThe Division of Spe cialized Care for Children (DSCC) helps sponsor the conference and serves on its steering committee.\nDSCC may be able to fund the conference-relate d expenses for DSCC participants and their families\, caregivers\, nurses and/or personal attendants. These expenses include the cost of the hotel a nd transportation.
