DESCRIPTION:Workshop series for anyone with a chronic health condition or t
 heir caregivers.\n\nFree six-week class meets every Monday evening startin
 g Aug. 20 - Sept. 24 from 6 to 8:30 p.m.\n\nLearn how to build a more eff
 ective relationship with your doctors\, problem solving and eating tips. 
 Participants will also explore an exercise routine\, communicate with fam
 ily and friends that share the same conditions\, learn about managing med
 ications\, work on setting and achieving goals and discuss dealing with p
 ain\, frustration\, isolation\, lack of energy.\n\nDeveloped at Stanford U
 niversity\, this evidenced-based self management program has produced the 
 following outcomes:\n\n 	Reduction of days in the hospital\n 	Reduction of
  emergency room visits\n 	Increased feeling of control of your life\n 	Imp
 roved quality of life\n 	Increased energy levels\n 	Significantly improved
