 nity members to learn about housing options for people with disabilities i
 n Illinois\n\nConnect to Community\, in partnership with the Village of Ho
 ffman Estates Commission for People with Disabilities and the Northwest Su
 burban Special Education Organization (NSSEO)\, invites you to the Transit
 ion Summit Focus on Housing on April 15 from 9 a.m. to 3 p.m.\n\nThis dayl
 ong event will provide information about a variety of housing options:\n\n
 Topics will include:\n\n 	Housing Options in Illinois: Community Integrate
 d Living Arrangement (CILA)\, Supportive Housing\, Intermediate Care Facil
 ities (ICF) and more.\n 	An Introduction to Host Family CILAs and EP!C - T
 heir Host Homes Program\, one of the largest in Illinois\, matches adults 
 with intellectual and developmental disabilities (I/DD) with host families
  in their community of choice.\n 	One Family's Story: Making Independent L
 iving a Reality - Parent Vicki Niswander will share her family’s experie
 nce creating a home for their daughter.\n 	Creating Housing Options: The N
 ew Futures&#x2122\; Initiative - a step-by-step process giving families su
 pported living options in their own neighborhood.\n 	Independent Service C
 oordination (ISC) Agency Statewide Housing Navigator Pilot - A program des
 igned to cultivate and pursue community-based\, independent housing option
 s for people with I/DD who are already in the waiver or newly selected fro
 m the PUNS list.\n\nThe registration fee includes morning refreshments\, l

 on fee with five hours of continuing education credits/professional develo
