DESCRIPTION:Annual summit will cover employment\, housing\, advocacy and mo
 re to help families of individuals with special needs plan for the future.
 \n\nConnect to Community\, in partnership with the Village of Hoffman Esta
 tes Commission for Disabled Citizens and the Northwest Suburban Special Ed
 ucation Organization (NSSEO)\, announces its fifth annual Transition Summi
 t. Topics to be covered include employment\, housing\, advocacy and more.\
 n\nFamilies\, educators\, service providers and community members are welc
 ome to attend the summit on April 13 at the Hoffman Estates Village Hall.\
 n\nThe agenda includes:\n\n 	Creating a Vision for the Future: Kelly Stumm
 e and Caren Musembi\, Ray Graham Association\, will talk about what famili
 es should consider as they plan for the future and how various residential
  options can fit into that plan.\n 	Supportive Housing: Julie Nelson\, Cor
 poration for Supportive Housing (CSH)\, will explain what Supportive Housi
 ng is\, how to access it through the Statewide Referral Network\, and then
  how to obtain the additional person-centered services needed for success 
 using CILA\, Home-Based\, or private funding. David F. and Clearbrook’s
  Jennifer Robinson will discuss David’s experiences finding his own apa
 rtment using the Supportive Housing model.\n 	Community Integrated Living 
 Arrangements (CILA): Usually thought of as community-based group homes\, C
 ILAs can take many forms. Learn about several CILA models\, including tra
 ditional and family-owned\, as well as options for those with higher medic
 al or behavioral needs.\n 	Family-Driven Creative Models: Two area nonprof
 its\, Our House Communities (ourhousecommunities.org) and Project ALEX Com
 munities\, Inc. (projalex.org)\, will discuss their efforts to create new 
 and different housing options for people with disabilities.\n\nThe $20 reg
 istration fee includes morning refreshments\, lunch  and seminar materia
