DESCRIPTION:&nbsp\;\n\nA virtual conference for physicians\, healthcare pro
 viders\, youth and young adults who have a chronic illness or disability a
 nd their parents/guardians\n\nJoin Baylor College of Medicine\, Division o
 f Continuing Professional Development\, for the "23rd Annual Chronic Illne
 ss and Disability Conference: Transition from Pediatric to Adult-Based Car
 e" on Oct. 27-28 from 8 a.m. to 4 p.m.\n\nThis virtual conference is for p
 hysicians\, healthcare providers\, youth and young adults who have a chron
 ic illness or disability and their parents or guardians. It aims to help t
 hem build skills\, prepare and plan for healthcare transition for children
  and youth with special healthcare needs (CYSHCN).\n\nOne of the major hur
 dles to CYSHCN receiving successful transition care to adult health care\,
  work and independence is the lack of a workforce trained to provide the n
 ecessary services.\n\nThis conference will help build skills and address t
 he importance of health literacy in the transition from pediatric to adult
 -based care. Participants will explore a variety of medical\, legal\, fina
 ncial and ethical issues\, including:\n\n 	How to improve mental health ou
 tcomes in individuals with intellectual disabilities\n 	Advances in health
  care transition (HCT) services for adolescents and young adults with psyc
 hiatric illness\n 	The legal and ethical issues involved in HCT for adoles
 cents and young adults with special healthcare needs\n 	Medication adheren
 ce using motivational interviewing\n 	The challenges and advantages around
  the treatment of anxiety and depression during the transition from pediat
 ric to adult-based care\n\nParticipants will be able to meet and talk with
  faculty in small groups\, exchange ideas and share knowledge and informat
 ion about how best to plan for a successful transition to adulthood.\n\nSe
