DESCRIPTION:&nbsp\;\n\nThe Arc of Illinois' lunchtime webinar will explain 
 DSCC's mission and care coordination.\n\nThe Arc of Illinois is hosting a 
 lunchtime webinar featuring the University of Illinois at Chicago's Divisi
 on of Specialized Care for Children (DSCC) at noon on Feb. 12.\n\nPresente
 rs for this webinar include:\n\n 	Thomas Jerkovitz\, Executive Director\, 
 has led DSCC since 2009. He has a distinguished career in healthcare admin
 istration within the state of Illinois and in the private healthcare indus
 try.\n 	Molly Hofmann\, APRN\, MSN\, PCNS-BC\, Associate Director of Care 
 Coordination\, oversees the administration for the development and impleme
 ntation of all care coordination operations\, policies and procedures for 
 the Core and Home Care programs.\n 	Darcy Contri\, RN\, BSN\, MPH\, Title 
 V Program Transition Specialist\, has provided support for transition-age 
 youth and their families since 1998.\n\nDSCC is a statewide program that 
 serves children and youth with special healthcare needs and their families
 . DSCC staff help families find the specialty care and resources they need
  for their child to reach their full potential.\n\nWe draw on the relation
 ships and expertise our staff has built since 1937 to address a child’s 
 medical\, social\, behavioral\, educational and financial needs. We do thi
 s through a process called care coordination\, which aims to meet the need
 s of the child and family as a whole. This support helps families feel mor
 e confident and organized in their child’s care and become well-prepared
  for the transition to adulthood.\n\nThe webinar will discuss DSCC’s el
 igibility requirements and examples of how our care coordination can benef
